Let the adventure begin…
(1) Medical Form
parent/Guardian/Adult Participant
This form must be filled out with the participant’s information. You will need the following to complete this form:
- List of medications, including dosage, side effects, restrictions, schedule, and for what condition
- Immunization records, including year administered
- Dates of significant injuries and illnesses
- Primary Physician contact information
- Health Insurance information (Policy Number, Group Number, Policy Holder, etc.)
(2) TERMS OF AGREEMENT

Please read the Terms of Agreement carefully! When you are finished reading, you must sign in the indicated areas. Please note, all tuition based information does NOT apply to this course as GOCO is covering all costs.
(3) RISK ACKNOWLEDGEMENT
All adult participants must fill out the Adult Risk Acknowledgement form.
